A mum attends with her son who is clearly distressed and in pain affecting the LLE. The mum refuses for her son to have an extraction. How would you manage this clinical case?

- Firstly, a thorough examination should be carried out and all options presented to both the child and their guardian.
- Try and understand why the mother doesn’t wish for an extraction. Explain the risks and benefits of all options and present these forward. Sometimes, patients are misinformed and when educated, they agree with the treatment options presented.
- If extraction is the only option and the mum refuses and you notice any signs of neglect or safeguarding red flags, raise this with your safeguarding lead.
- Try and ensure that the patient is not in undue pain as well as agreeing an appropriate follow-up plan is implemented in the event that initial consent is not agreed.
- Always ensure you take good notes/record keeping.
