William comes into your pharmacy saying he is hearing voices and refuses to take his tablets as a result. He comes to speak to you, the responsible pharmacist, as he is concerned and needs some advice and wishes for the voices to go away. He tells you that he is prescribed medicines, however he is not a registered patient at your pharmacy and no records of previous supply can be found on your pharmacy management system. With informed consent, he grants you access to his SCR. You verify the Williams identity, and glean he is taking Hyoscine hydrobromide 150mcg 2 at night, Amlodipine 5mg tablets 1 at night, Rivaroxaban 20mg tablets 1 OD. You later learn by way of conversation that William takes clozapine 125mg OD. How would you handle this patient? What are your concerns and how would you manage them?
