Mrs Jones is a 65 year old lady who suffered a myocardial infarction almost six months ago. She was prescribed clopidogrel to take once daily. She also has indigestion and takes omeprazole. You are conducting a Medication Use Review with her and she informs you that she does not take clopidogrel daily as she does not see or feel any benefit from it. However, she takes omeprazole every day as it helps relieve her symptoms. Explain what points you would cover during the MUR and identify any course of action you may need to take to optimise her medication.
Example Answer
Patient Understanding: Establish how often Mrs. Jones takes clopidogrel and her understanding of the reasons why she needs to take this medicine. Explain the risks of not taking clopidogrel. It is important to use language that is easy for the patient to understand.
Polypharmacy: Identify the interaction between omeprazole and clopidogrel and explain this to the patient in simple verbal communication that she may understand. Thereafter it would like be appropriate to make an action plan to switch omeprazole to another non-interacting PPI .
Lifestyle Measures: Discuss lifestyle measures she may take to reduce the risk of further heart attacks and reduce symptoms of indigestion (i.e. diet, exercise).
