Mrs Hopper comes to your pharmacy with a prescription for Prednisolone enemas for ulcerative colitis. She tells you that she has recently been diagnosed with the condition and that this is her first prescription for an enema. She says she would rather have tablets, but the doctor suggested that an enema would be more appropriate for her. Explain what you would discuss with the patient.
Example Answer
Medication Indication: Explain what prednisolone is and how it works in helping treat her symptoms of ulcerative colitis.
Patient Concerns: Discuss with the patient her concerns about using the enema, and address these concerns wherever possible, also explaining why she may have been given the enema rather than tablets (more locally effective with reduced systemic side effects).
Counselling: Counsel the patient on how to use her enema and its side-effects. Corticosteroids (with long term use) can cause increased appetite, weight gain, insomnia, depression, osteoporosis, peptic ulceration, and glucose intolerance.
Lifestyle Advice: Discuss how her diet may affect her condition and how she can modify her current diet to help her symptoms and reduce the risk of flares; considering the benefit of offering official advice from organisations such as Crohn’s & Colitis UK etc.
